Fixed Implant Supported Prostheses | Dr. Peter Taylor | Shore Side dentistry

Article Abstract:

This article describes the use of the ITI implant system in rehabilitation of the maxillary anterior sextant of a 41-year- old patient. The Syn-Octa TS abutment system was used in this case, along with a customized transfer aid for intraoral positioning of these elements. This system offers the advantage of retrievability of the prosthesis in multiunit cases.

Key Innovation: Retrievable Fixed Prostheses

The Problem Solved: Traditional fixed implant prostheses are difficult to remove once cemented, making adjustments, repairs, or maintenance extremely challenging and costly.

The Solution: The Syn-Octa TS (Transverse Screw) system allows for retrievable fixed prostheses – giving patients the stability of fixed teeth with the flexibility of removable maintenance.

Case Overview

Patient Profile

  • 41-year-old woman with failing 6-unit bridge
  • Root fractures requiring extraction of supporting teeth
  • Sensitive gag reflex – needed fixed solution, not removable denture
  • Esthetic concerns in the visible front teeth area

 Treatment Challenge

Restoring 4 missing front teeth while maintaining:

  • Natural appearance
  • Full function
  • Easy maintenance access
  • Long-term retrievability

The Syn-Octa TS Advantage

Why This System is Superior

🔧 Retrievability Benefits:

  • Prosthesis can be removed for repairs without damaging implants
  • Easy access for thorough cleaning and maintenance
  • Cost-effective adjustments and modifications
  • Reduced chairside time for both patient and dentist

Technical Advantages:

  • Transverse screw retention – screws enter from the side (lingual), not through the biting surface
  • Esthetic access holes – hidden on the back of teeth, not visible when smiling
  • Precise positioning using custom transfer aids
  • Superior fit with minimal marginal discrepancies

Treatment Process Highlights

Advanced Techniques Used

  1. Custom Transfer System
  • Fabricated combination Duralay and transfer aid assembly
  • Ensures precise positioning from laboratory model to patient’s mouth
  • Eliminates guesswork in abutment placement
  1. Provisional Testing
  • Temporary prosthesis created first to test function and esthetics
  • Patient input incorporated before final fabrication
  • Optimal occlusion and phonetics established
  1. Laboratory Collaboration
  • Patient attended laboratory for custom staining
  • Ensured perfect color matching and natural appearance
  • Coordinated approach between dentist and technician

Clinical Outcomes

Immediate Results

Perfect esthetic integration in the smile zone
Optimal function with proper occlusion
Patient satisfaction with appearance and comfort
Successful retrievability demonstrated at delivery

Long-term Benefits

  • Reduced maintenance costs due to easy access
  • Predictable repairs when needed
  • Better periodontal health through improved cleanability
  • Prosthesis longevity through proper maintenance capability

Clinical Significance

Why This Matters for Patients

Traditional Cemented Prostheses:

  • Difficult to clean thoroughly
  • Expensive to repair or modify
  • Risk of cement residue causing gum problems
  • Often require complete remake for adjustments

Syn-Octa TS System:

  • Easy maintenance and cleaning
  • Cost-effective repairs and adjustments
  • No cement-related complications
  • Modifiable without complete remake

Professional Impact

This case demonstrates how advanced implant systems can provide:

  • Better patient outcomes through improved design
  • Reduced long-term costs for both patient and practice
  • Enhanced treatment predictability with proven techniques
  • Superior esthetic results in challenging anterior cases

Key Takeaway

Dr. Taylor’s case study proves that retrievable fixed prostheses represent the future of implant dentistry – combining the stability patients want with the maintenance access clinicians need for long-term success.

The Syn-Octa TS system addresses the fundamental challenge in implant prosthetics: How do you provide permanent-feeling teeth that can still be properly maintained over time?
